Job Overview

The Clinical Manager is responsible for the oversight of client care services within the home healthcare setting. This role involves managing and supervising clinical staff, ensuring compliance with healthcare standards and regulations, coordinating client care plans, and fostering effective communication between healthcare providers, client, and families. The Clinical Manager works to ensure that client care is delivered in a safe, compassionate, and efficient manner.

Essential Duties

The unique responsibilities for this role include but are not limited to the essential functions listed as follows:



  • Provide supervision of home health aides in the provision of client care.

  • Monitor the coordination of customer care services.

  • Conduct initial and on-going client assessments and communicate findings to Executive Director (ED).

  • Review and develop the Client Care Plan.

  • Identify the on-going needs of all clients to maintain their health and wellness.

  • Be on call 24/7 for client/family questions of a medical nature.

  • Monitor all infections/ incidents of clients. Monitor compliance of all infection control policies.

  • Consult with physician when necessary to ensure and maintain safe, efficient, and quality client care.

  • Keep and maintain record of case assignments and case management.

  • Maintain clinical files electronically and/or in hard copy as designated and ensure proper documentation.

  • Review all client or team member incident reports and develop a plan of correction to increase safety for both clients and team members.

  • Review the accuracy of all physician orders quarterly and as needed and ensure that physician orders are current for each client.

  • Provide consultation and technical assistance to team members as needed.

  • Coordinate client service needs with other team members as well as with outside community providers.

  • Maintain communication with the client’s family to ensure that all medical needs of each client are met.

  • Contact hospital or other facilities at which clients are temporarily residing regularly to obtain updates on their condition.

  • Develop a working knowledge of all home care policies and procedures.

  • Participate in the quarterly medical record review process.

  • Conduct client home visits every 60 days or as needed based on clinical changes.

  • Provide in-house Orientation Training Class, In-Services, and Home Health Aide Certification (CHHA) Training Class.

  • Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Dignity standards are always met.

  • Perform other duties as assigned.

Experience and Qualifications


To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed throughout this job description are representative of the knowledge, skills, and abilities required.



  • Bachelor’s degree in Nursing (BSN) or related healthcare field; Master's degree preferred

  • Current Registered Nurse (RN) license in the state/province of operation

  • Minimum of 3-5 years of clinical experience in home healthcare or a related field, with at least 2 years in a supervisory or management role

  • BLS certification required; additional certifications (e.g., CHPN, OASIS) preferred.

  • Knowledge of home health regulations, Medicare/Medicaid rules, and clinical standards
  • Free from any mental or physical health problems that may be injurious to the client and team members as they perform their delegated tasks
